"Yuji" bulbs and "waveform" bulbs seem to both fit all your specs. Both have full tests thanks to maukka. Only potential drawback, ~20$ per bulb.
For such lumens, you can ask some custom makers, like "SunLike" to build something for you. He can go up to do 50W LED bulb from high CRI COBs. Drawback, the BLF test is not done yet, it will be in a few days/weeks. Might be a bit pricey too.
All those candidates are quite detailled (with links to manuf. and tests, etc) in the thread I refered to in my first sentence.
You might also have results by searching D50 calibrated bulbs. But, still, they will be pricey.
Which application is making you specifically look for 5000K ligts?
KinoFlo would meet the specs too but they aren’t cheap. Possibly into the $1000-$2000 range. I’m too lazy to look up the prices.
I’ve seen the KinoFlo lights in demos. They look very good. There are others too but pro movie lighting is waay outside of my work and personal needs so I don’t pay much attention to it.
Just realized there is another corn bulb with great potential for your #2 case : Marswalled B07PNVTPCR. It currently does not look so great in the BLF table, but this is due to the fact that its R12 value is untested yet. So its comparison is a bit biased. For its "3000 claimed lumens" its price is nice.
