True - a solder blob would be a ‘manufacturing flaw’.
It’s also possible to have a ‘design flaw’ - mismatching battery and load resistance.
And all sorts of ‘operational errors’ - letting it vape too long, overdischarging a battery, using the wrong battery.
All these news reports should be saying that modifying vape stuff is WAY more dangerous than using a flashlight or a cell phone, where either the situation is less strenuous (most lights) or more controlled (cell or laptop, or electric car.)

I have about 20-some mechanical vape devices. There usually isn’t soldered parts or wires in a true mech mod, only machined parts. Hence the name mechanical.
More likely a cheap crap ultrafire or something, possibly in a mech, Never once heard of a vent or explosion while using brand name cells like LG, Samsung or Sony. But yeah, there’s always that “Cloud Bro”, that pushes the limits of resistance.
