I cant tell you much other than its about 5w and 12v forward voltage and its mounted on a 20mm MCPCB.

I have no idea, it is very small for what looks like a COB led, only about 5mm diameter.
I dont recognize this LED
Are you sure its a 12V LED or just the light itself is for 12V?
if its true 12V it should have 4 separate LEDs inside and its forward voltage should be around 12-12.5V at 5W
but for 5W 12V LED a Cree XT-E High-Voltage XHP35, or XHP50 on a 20mm star should be a good replacement

You can just about make out there is several very small LEDS behind the phosphor layer
