More info needed. How custom? Convoy has a very good selection of reflectors and lenses. Smooth reflectors can be made OP by diffusion film on the glass.

Got it! I think it would be preferable to work backwards: find the parts (LED, reflector, pill, etc.) that you want to use, and design your light around them. Doing so avoids a ton of hassle and extra cost.
Kaidomain has a freakton of reflectors (225) and lenses (147) to choose - you will probably find something that fits 
At a smaller scale, Kaidomain and MtnElectronics both have parts for building your own light. Convoy has some parts as well, if you’re already familiar with their lineup. For larger scale manufacturing, you might have luck on Alibaba for finding larger quantities, though since they’re M2M, rather than M2C, it’s trickier ordering, as you may have to deal with taxes, freight, and all the stuff associated with that.
The go-to usa provider of custom glass for relatively low prices is Abrisa. For small batch production and prototypes I grind my own glass with a CNC grinder.
For usa electroformed reflectors, Phoenix electroforms, but electroformed reflectors are somewhat costly and they do not do OP coating. Custom aluminum reflectors can be turned on any turning center, or better, via single point diamond turning.
Thomasnet is a resource for finding product suppliers. Alibaba is another.
You can take virtually any existing reflector and modify it to the required size via machining. The same is true of glass windows.
There is almost always an MOQ for custom work, or at least the setup fee is such that you basically get the parts for free up to a certain qty, if that makes sense.
