I was shocked to see my trustfire x6 turned on with the tail cap. I was installing fresh new batteries after i put in the 3 cell it turn on without the tailcap.
Unless theres a short somewhere, that would be shocking indeed. Can you take a video of this happening?
your 3rd cell is not insulated/the wrapper is damaged

No.
It’s best not to have that cell in your light until you at minimum re-wrap it.
The torn area has already produced a short. And depending on the depth of the cut, it may begin to leak. A leaking battery will damage the light, particularly, if it gets down into the driver area.
Batteries are cheap and easy to change, compared to having to replace a driver.
If you have any doubts as to the success of a re-wrap just replace it
